Time Out says

Men at this traditional festival let their feminine side shine by donning wigs, make-up and brightly coloured outfits and stepping out as women for the day. As well as dressing up, participants entertain the crowds by dancing and scattering paper charms. In the Edo era, you could find similar festivals in Osaka and Edo (present-day Tokyo), but now it's confined to Yokohama's Totsuka ward. Much like the infamous Kanamara Matsuri 'penis festival', held in nearby Kawasaki, expect plenty of TV crews and amateur photographers.
